What must be linked before an AI content report is closed.

Required Checks
- The report or issue ID is recorded.
- The report is linked to the affected content.
- The content class is recorded.
- The source object is identified where available.
- The generation record is linked where available.
- The output and source snapshots are preserved where available.
- Issue category and severity are recorded.
- Reviewer role, decision, rationale, and outcome are recorded.
- Reporter communication status is recorded where contact details exist.
- A correction or product change is linked if one was made.
- Escalation status is recorded.
- A transparency-log decision is recorded where the correction is public or material.
- Restricted evidence references are retained outside the public render path where needed.
Material Closeout Standard
Material closeout should be understandable to a later reviewer without relying on chat history. It should show what was reported, what source or generated output was checked, what decision was made, what action changed, why public logging was or was not needed, and whether legal or senior review was considered.
Public Boundary
Public correction records should explain the issue and outcome without exposing private user details, private reviewer notes, or sensitive incident evidence.
